If your team is in need of opening a team bank account or making changes to the your current team account, you will require a letter from the SPDSA in order for the banks to open an account for your team. Please fill out the following form and hit submit. The form must be filled out completely in order for a letter to be generated. Once your letter has been issued, you will be notified via email that its ready to be picked up at the SPDSA office. Letters must be picked up at the office and email copies will not be issued. Team bank accounts are mandatory for ALL Phoenix and Jr Phoenix Teams.
If your team is changing signing authorities, please indicate on this form and a letter will be issued for your team.
