Trialist Request Form

If your team is in need of a Trialist for an upcoming game or tournament, please contact the coach of the team that you require players from. Once the players have been confirmed, the coach that is requiring players will fill out this form. We still require 3 days notice if player cards are required. Trialists for EIYSA games, must have a current EIYSA card and EIYSA requires a minimum of 3 business days in order to have a Trialist card done.
