Provincials Application Form

If your team is interested in attending Provincials for the upcoming season, Phoenix FC will need the following application filled out.

Recreational Tier 4 Teams - The highest placed team that declares for Provincials will represent Sherwood Park at Tier IV Youth Provincials*
Competitive Tier 1-3 Teams - All Tier 1-3 teams must also fill out the EMSA form. Phoenix FC does not have a automatic birth, All Tier 1-3 teams must earn their sport through the EMSA League.
Adult Teams - Phoenix FC does not send teams through provincials from our in house leagues

All 7v7 and 9v9 teams must play 6 games in boarded to qualify for Indoor Boarded provincials, please indicate tournament entered or exhibition games played. (Game Sheets must be shown and provided to SPDSA)

How Many players do you have confirmed for Provincials?

You agree that your team will comply with Rule 10.1 in the ASA Provincial Rule Book and will be responsible for any fees sanctioned to the SPDSA: $500 Fine for withdrawing 15-30 Days before Provincials and $2,500 for withdrawing less than 14 days before Provincials.

You agree that your team will comply with Rule 10.2 & 10.3 in the ASA Provincial Rule Book and will be responsible for any fees sanctioned on the SPDSA: "Any team failing to attend all scheduled games will be fined an administration fee up to $2,500"

You acknowledge that the SPDSA Technical Department will have the final decision on the all team's eligibility for Provincial Competitions.
