If your team is doing a Team Fundraiser, Team Event, Exhibition Game or Tournament, please fill out this online form. Teams that will be participating in Exhibition games, must ensure that all players are ASA insured and are eligible to play.

Once your event has been reviewed, you will receive an email confirming approval. Please allow sufficient time for your application to be processed.

Please note that for teams traveling outside of Alberta, they MUST fill out the ASA Travel Permit and email it to our office at office@spdsa.net
